Overview

The ITAM Admin Portal Form Builder has an extensive array of Field Types designed to help the Administrative User design a form that captures the data their organization needs in the most efficient and effective way possible. To get the most out of the effort put into building forms, it is important to understand the various types of fields, what their purpose is, and what options they provide to fine tune their use.

The Form Builder comes with 22 different field types to assist the Administrative User in designing a form that will best meet their oversight needs. Some of the fields exist to provide information to the user, either about the form or about the process. Others exist to help manage and control the flow of the form. Many exist to capture and validate a specific type of data and some include the ability to weight the answers to simplify compliance validation.

Shared Features

Many, and in some cases all, fields share some features in common.

Most field options have a help link next to the name of the field to provide information about the field and its intended purpose and use. Simply click on the small icon of a question mark and some information will pop up on the screen about the field.

Rich Text Fields

Many of the larger text field properties allow for rich text. Rich text fields allow for greater formatting options than simple text fields and provide buttons to simplify common formatting needs.

Data Entry Fields

Simple Data

Single Line Text

This field is a great choice when you have just a single line of text information you want to acquire from the respondent in your form. A single field will accept up to 65,535 characters or about 18 full pages of text.

Text Option

Display as Password Field

Checking this will display the field as a Password Field and mask all the characters (shown as asterisks or circles). Please be aware that there is no encryption being made for the password field. You will be able to see it from the admin panel/email as plain text.

Limit By

You can limit the amount of characters typed to be between certain characters or words, or between certain values in the case of a Number Field. Leave the value blank or 0 if you don’t want to set any limit.

Paragraph Text

This field is a great choice when you have a paragraph of text information you want to acquire from the respondent in your form. A single field will accept up to 65,535 characters or about 18 full pages of text.

Time Options

Show Seconds Field

Checking this will enable the Seconds field on your time field.

Use 24 Hour Format

This will enable 24-hour notation in the form hh:mm (for example 14:23) or hh:mm:ss (for example, 14:23:45)

Date

This field is a great choice when you want to include the acquisition of a date from your respondent.

Date Options

Enable Minimum and/or Maximum Dates

You can set minimum and/or maximum dates within which a date may be chosen.

Enable Date Selection Limit

This is useful for reservation or booking forms so that you could allocate each day for a maximum number of customers. For example, setting the value to 5 will ensure that the same date can't be booked/selected by more than 5 customers.

Disable [Past|Future] Dates

Checking this option and picking “past” or “future” will disable the selection of either past or future dates respectively.

Disable Weekend Dates

Checking this option will disable all weekend dates.

Disable Specific Dates

You can disable any specific dates to prevent them from being selected by your clients. Use the Date Picker to disable multiple dates.

Number

This field is a great choice when you want to collect or display numbers to your form respondent. You can limit the number of characters typed to be between certain characters or words, or between certain values in the case of a number field. Leave the value blank or 0 if you don't want to set any limits.

Advanced Options

Enable as Quantity field

Enable this option if your form has payment enabled and need to use the quantity field to calculate the total price. When checked, it will display an option to select the target field for the calculation from a dropdown list. Target field type must be one of the following: Multiple Choice, Drop Down, Checkboxes, Price.

Price

This field is a great choice when you want to include currency fields.

This field is a great choice when you want to allow the form respondent the ability to upload files into ITAM.

Upload Options

Limit File Upload Type

You can block or only allow certain file types to be uploaded. Choose “Block” to block only those file types entered. Choose “Allow” to only allow the file types entered. Provide a comma-separated list of file type extensions to which you want to apply the rule.

Send File as Email Attachment

By default, all files uploaded will be sent to your email as a download link. Checking this option will send the files as an email attachment instead.

Don’t enable this option if you expect to receive large files from your clients. If the files attached are larger than the allowed memory limit on your server the email won’t be sent.

Enable Advanced Uploader

Checking this option will enable advanced functionality such as an Upload Progress Bar, Multiple File Uploads, AJAX uploads, File Size Limit, etc.

It is recommended that you enable this option.

Enable Synced Upload

Checking this option will enable Synced File Upload.

Advanced Uploader Options

Automatically Upload Files

By default, the Upload button or the Form Submit Button needs to be clicked to start uploading the file. If this option is checked, a file will automatically start uploading as soon as the file is selected for upload.

Allow Multiple File Upload

Checking this option will allow multiple files to be uploaded. Use the associated text box to limit the number of files allowed to be uploaded.

Limit File Size

Checking this option will limit the maximum size of a file that can be uploaded to the value set in the associated text box.

Allow Selecting Files Previously Uploaded

Checking this box will allow entity users the option to select a file previously uploaded into any form owned by that entity instead of uploading a new file.

This field is a great choice when you want to include the collection of address information.

Checking this will limit the country selection to the United States only and the state field will be populated with U.S. state list

Checkboxes

This field is a great choice when you want to present a variety of choices in a question format where any or all of the responses may be selected. Question fields allow you to create as simple or as complex a questionnaire field as needed. Use the plus and minus buttons to add and delete choices. Click on the choice to make it the default selection. Please enter a numeric value in the field adjacent to the response that may be used for risk-weighting or other analytical calculations of your choice.

Choice Columns

Set the number of columns being used to display the columns. Inline Columns means the choices are sitting next to each other.

Question fields allow you to create as simple or as complex a questionnaire field as needed. Use the plus and minus buttons to add and delete choices. Click on the choice to make it the default selection. Please enter a numeric value in the field adjacent to the response that may be used for risk-weighting or other analytical calculations of your choice.

Matrix Options

Allow Multiple Answers Per Row

Checking this option will allow your client to select multiple answers for each row. This option can only be set once when you initially added the matrix field. Once you have saved the form, this option can't be changed.

Dropdown

This field is a great choice when you want to present a variety of choices in a question with a drop-down style format where only one of the responses may be selected. Question fields allow you to create as simple or as complex a questionnaire field as needed. Use the plus and minus buttons to add and delete choices. Click on the choice to make it the default selection. Please enter a numeric value in the field adjacent to the response that may be used for risk-weighting or other analytical calculations of your choice.

This field is a great choice when you want to display formatted messages such as RSS feed information within your form displayed to the respondent. It's a great way to display advertisements or other important information as many times within a form that you require.

Media Player

This field is a great choice when you want to present a variety of media selections such as MP4 videos, external URL-based links to video content, MP3 audio content, and image files such as PNG, JPG, and GIF files. Display attributes are automatically scaled to the form's width but a pop-up window feature allows for full screen viewing too. Display training content, advertisements, instructional, and informational content, or anything relevant to your current module's form content for a rich multimedia experience.

Section Break

This field is a great choice when you want to divide your form up into sections that appear all on the same page. Options to include preformatted text is present. A popular use of this field is for narratives and informational sections in a form making it more descriptive and meaningful to your respondents.

Admin Only

Fields that are set to 'Admin Only” will not be shown to users when the form is made public

Section Break Options

Display Section Break in Email

Enable this option if you need to display the content of the section break within the notification email, review page, and entry page.

Enable Scrollbar

The section break will be set to a fixed height and a vertical scrollbar will be displayed as needed. This is useful to display a large amount of text, such as terms of agreement or contract agreement.

Page Break

This field is a great choice when you want your form to have multiple pages. There is no limit to how many pages you may have in a form as long as they are not blank pages.

Background Options

Background Color

The background color of the Submit Button expressed as an HTML Color code. A Color Picker will be presented to facilitate the selection of an appropriate color.

Cascade Form

This field is a great choice when you want to connect multiple forms creating a contiguous experience for your respondents. If you want to simplify complex forms with common templates that you "stitch" or link together, this is a powerful innovation that allows you to do that.

When you place this field into position anywhere in your main form you are presented with a multiple-choice drop-down selector containing all the active forms in your current inventory. You can cascade multiple separate forms together. You are also able to cascade within a cascaded form creating complex form nesting scenarios. The form fields will assume the name of the cascaded form name present in the drop down form selector.

The results of the respondent entries will stay with the specific forms cascaded together quite elegantly.

Any form that is connected by a cascade field will independently update data when a user enters information and in addition to the separate form data set, the updates flow upwards to the top-level cascade form data set. Bi-directional data management is possible with the top-level collation of all separate form data into one unified experience and report output.
